Edit 3: The solution turned out to be to install the latest GOG release from CD instead of the vanilla release from CD with 1.29 patch. I'm not sure why, but I can't get the vanilla release to run at all in any resolution.
Edit 2: I also tried the full-screen mode with the keepres set to 4 instead of 1. That is the lowest setting possible and it still didn't work. So, I'm not sure if that is the best option or not.
Edit: If you are playing on a system with the same specs, you might want to try the CD's with each option enabled and running at the same time. (ie. full screen exclusive mode and keepres) and then select the one that runs best. As I stated, the keepres with the full-screen exclusive mode runs the best.
When I play the game in full-screen exclusive mode with KeepRes=1, the menu is visible, and the game itself works fine. But it produces a huge hiccup when it attempts to load into the menus. Instead of moving smoothly, it jumps back and forth as if something was colliding with the screen.
I have tried looking at the resolution in game by looking at the window properties and it seems to be maintaining the same resolution in both modes. When in full-screen exclusive mode with KeepRes=1, the screen looks very dark. When playing the game in full-screen windowed mode with KeepRes=1, I can see the lower part of the screen although the black bar where the intro video is is still visible.
